Day Services consist of a daily schedule structured to the individual’s interests and personal preferences.  The day program also consists of various groups as described in the clinical. There is close communication between the providers of primary program support services and/or program options and the adult consumer’s family, guardian, and/or primary caregivers to assure integration of services.

APT advocates for and works with individuals and their families to access services such as higher education and/or vocational training programs, as well as offer a variety of community-based primary programs for Individuals. This includes opportunities for Individuals to participate in self-advocacy, employment training, community integration, and the development of self-care skills.   Primary programs for Individuals will provide the opportunity to Individuals providing them with planned activities to do outside of their home environment for a portion of each day.  

Clinical and Behavioral services are offered in-house.  Individuals are able to connect with APT Clinical Director as needed.  APT works with community therapists and psychiatrists who are providing services to individuals in order to create an effective continuum of services.  APT provides daily clinical oversite of all individuals and staff Clinical and Behavioral supports are offered 24/7.   A variety of weekly groups are offered to individuals who are able and willing to participate in group therapies.  

APT has a Nurse Trainer who oversees all medication and medication changes. The Nurse Trainer oversees all He-m 1201 NH State Regulations and provides and certifies training to staff, HCP,s and also the individuals if deemed appropriate.   Quality assurance checks are completed monthly.  

APT makes every attempt to assist individuals in achieving greater independence.  APT provides intense clinical support to individuals who are transitioning from an HCP setting to independent living.  Nursing and Clinical support are offered and available 24/7.
